Privacy Patient Sign In Sheets. 

Privacy sign in sheets-  You remove the strip with the patients signature once the patient has signed-in.  This protects the patient's privacy.  Some doctors use this strip in the chart as proof the patient kept the appointment. 

We have several colors available, however, I ended up with several cases of the "Salmon Color" pinkish sign in sheets.  We are offering this color for HALF-PRICE until the inventory is gone.
